This practice, called antibiotic prophylaxis, is a hot topic in the medical and dental worlds. It involves taking antibiotics before certain procedures to prevent a rare but serious infection called infective endocarditis. Let's explore what this condition is, why we sometimes use antibiotics to prevent it, and who might need this extra protection.

What is Infective Endocarditis?


Infective endocarditis (IE) is an infection of the inner lining of the heart chambers and heart valves (the endocardium). It happens when bacteria or other germs enter the bloodstream and travel to the heart. These germs can then attach to damaged or abnormal heart tissue, forming a vegetation, or a cluster of bacteria and other cells. This vegetation can interfere with the heart's normal function and can even break off and travel to other parts of the body, causing serious problems like stroke or organ damage.

The symptoms of IE can vary, but they often include fever, fatigue, shortness of breath, and new or changed heart murmurs. IE is a serious condition that requires prompt treatment, usually with long-term antibiotics given intravenously in a hospital. In some cases, surgery may be needed to repair or replace damaged heart valves.

Why Use Antibiotics Before Dental Procedures?


Certain dental procedures can introduce bacteria into the bloodstream. While this happens frequently and usually doesn't cause any problems, in rare cases, these bacteria can cause IE in people with certain heart conditions. That's where antibiotic prophylaxis comes in. The goal is to kill any bacteria that might enter the bloodstream during the procedure before they have a chance to cause an infection in the heart.

Who Needs Antibiotic Prophylaxis?


The American Heart Association (AHA) has guidelines for who needs antibiotic prophylaxis before dental procedures. These guidelines are based on the best available scientific evidence and are regularly updated. Currently, antibiotic prophylaxis is recommended only for people with specific heart conditions that put them at the highest risk of IE. These conditions include:

•  Prosthetic cardiac valves or prosthetic material used for cardiac valve repair
•  A history of infective endocarditis
•  Specific congenital (present at birth) heart defects, including:
•  Unrepaired cyanotic congenital heart disease, including those with palliative shunts and conduits
•  Completely repaired congenital heart defect with prosthetic material or device during the first six months after the procedure
•  Repaired congenital heart disease with residual defects at the site or adjacent to the site of a prosthetic patch or prosthetic device

If you don't have one of these specific heart conditions, you most likely do not need antibiotics before dental appointments.

The Importance of Good Oral Hygiene


While antibiotic prophylaxis is important for those who need it, it's crucial to remember that good oral hygiene is the best way to prevent IE. Regular brushing, flossing, and professional dental cleanings can help keep your mouth healthy and reduce the number of bacteria in your bloodstream. This is true for everyone, regardless of whether or not they need antibiotic prophylaxis.
